Recognizing the insurer's name is only the beginning. Confirm that your doctor, specialists and hospital are in the network of the specific plan you intend to choose.
Have your doctors' names and office locations ready.
Check whether your prescriptions are covered, their costs and in-network pharmacies. Some may require prior authorization or have specific conditions.
Prepare a list with the name and dosage of each prescription.
Your premium is the monthly payment. The deductible, copayments, coinsurance and out-of-pocket maximum for covered services also matter.
Consider both routine visits and unexpected expenses.
Confirm your enrollment period and coverage start date. A life change may open a special opportunity, depending on the program and its requirements.
If you lost coverage, keep the document showing when it ended.
